Clinical Pharmacology:

This anticholinergic preparation blocks the responses of the sphincter muscle of the iris and the ciliary muscle to cholinergic stimulation, dilating the pupil [mydriasis]. The stronger preparation (1%) also paralyzes accommodation. This preparation acts rapidly and the duration of activity is relatively short . The weaker strength (0.5%) may be useful in producing mydriasis with only slight cycloplegia.

Indications And Usage:

Appamide Eye Drops is used to widen (dilate) the pupil of the eye in preparation for certain eye examinations. It belongs to a class of drugs known as anticholinergics. Tropicamide works by relaxing certain eye muscles.

Contraindications:

This product is contraindicated in persons with primary glaucoma or a tendency toward glaucoma (e.g. narrow anterior chamber angle), and in persons showing hypersensitivity to any component of this preparation.

Warnings:

For topical use only - not for injection. Reproductive studies have not been performed in animals.There is no adequate information on whether this drug may affect fertility in human males or females or have a tetratogenic potential or other adverse effect on the fetus. This preparation may cause CNS disturbances which may be dangerous in pediatric patients. Possibility of occurrence of psychotic reaction and behavioral disturbance due to hypersensitivity to anticholinergic drugs should be borne in mind.

PRECAUTIONS:

General

In the elderly and others where increased intraocular pressure may be encountered, mydriatics and cycloplegics should be used cautiously. To avoid inducing angle closure glaucoma, an estimation of the depth of the angle of the anterior chamber should be made. The lacrimal sac should be compressed by digital pressure for two to three minutes after instillation to avoid excessive systemic absorption.

Dosage And Administration:

For refraction, one or two drops of the 1% solution in the eye(s), repeated in five minutes. If patient is not seen within 20 to 30 minutes, an additional drop may be instilled to prolong mydriatic effect. For examination of fundus, one or two drops of the 0.5% solution 15 or 20 minutes prior to examination. Individuals with heavily pigmented irides may require larger doses.

DESCRIPTION:

APPAMIDE PLUS® Eye Drops (Tropicamide & Phenylephrine Hydrochloride Ophthalmic Solution) is a combination of a mydriatic/cycloplegic drug. Phenylephrine is a synthetic sympathomimetic (adrenergic mydriatic following topical application, phenylephrine brings about contraction of the radial muscle thereby bringing about dilatation of the pupil. Tropicamide is a parasympatholytic drug which inhibits the contraction of the circular muscle and also paralyses the ciliary muscle. Tropicamide functions as a cycloplegic/mydriatic drug.

INDICATIONS

APPAMIDE PLUS® Eye Drops (Tropicamide & Phenylephrine Hydrochloride Ophthalmic Solution) is indicated in Ophthalmoscopy and other procedures, where mydriasis is desired.

PHARMACO THERAPEUTIC GROUP

APPAMIDE PLUS® Eye Drops(Tropicamide & Phenylephrine Hydrochloride Ophthalmic Solution) are commonly used to dilate the pupils topical application ( Tropicamide - muscarinic receptor antagonist and phenylephrine - α- adrenergic receptor antagonist)

CLINICAL PHARMACOLOGY

Mechanism  of  Action
Tropicamide is a nonselective muscarinic antagonist. The relatively greater diffusibility of Tropicamide accounts for faster and shorter duration of action compared with other anticholinergic agents for maximum mydriasis occuring in 20 to 40 minutes after instillation. the maximum cycloplegic effect occurs within 30 minutes after instillation. complete recovery of accommodation and normalization of pupillary size occur is as little as 3 hours. Phenylephrine in APPAMIDE PLUS® Eye Drops (Tropicamide & Phenylephrine Hydrochloride Ophthalmic Solution) is a sympathomimetic amine which acts primarily on Alpha receptors. After topical application, phenylephrine contracts the iris dilator muscle and smooth muscle of the conjuctival arterioles, causing pupillary dilation. Phenylephrine is used for mydriasis at concentration between2.5% & 10 %

CLINICAL USE

Due to faster onset, shorter duration and sufficient intensity of action, Tropicamide is considered to be the drug of choice for Ophthalmoscopy and other procedures in which mydriasis is desired. Phenylephrine dilate the pupil from concentration above 5% has a little effect. the combination of Tropicamide and Phenylephrine allows the patients pupil to dilate with only one single drop. The combination is well tolerated by young children.

CONTRAINDICATIONS

APPAMIDE PLUS® Eye Drops (Tropicamide & Phenylephrine Hydrochloride Ophthalmic Solution) is contraindicated in patients with hypersensitivity to belladonna alkaloids, patient taking M.A.O inhibitors, reserpine, methyldopa, in narrow angle glaucoma patients, in low birth weight infants, in adults with severe cardiovascular disease, in intraocular operative procedures when the corneal epithelial barrier has been disturbed.

WARNING

If irritation persists or increases on using APPAMIDEPLUS® Eye Drops(Tropicamide & Phenylephrine Hydrochloride Ophthalmic Solution), discontinue the use and consult the Physician. Use the Solution within one month after opening the container.

PRECAUTIONS
Information for  Patients
Patients should be instructed to avoid allowing the tip of the dispensing container to contact the eye or surrounding structures. Patients should also be instructed that ocular solution scan become contaminated by common bacteria known to cause ocular infections. 

DRUG INTERACTIONS
The Mydriatic effect of Tropicamide can be completely reversed by thymoxamine. Dapiprazole reduces pupillary and accommodative recovery time and it may also interfere with the antiglaucoma drugs. Severe hypertension may occur when phenylephrine is administered concurrently with monoamine oxidase inhibitors, tricyclic antidepressants, beta-blockers and guanethidine. 

PREGNANCY
The safety of Tropicamide in APPAMIDE PLUS® Eye Drops (Tropicamide & Phenylephrine Hydrochloride Ophthalmic Solution) for use during pregnancy has not been established. There is a potential risk of fetal exposure to Tropicamide due to systemic absorption by the mother. The drug should be used only when the indication is clear and the potential benefit to the mother outweight the potential risk to the fetus. The safety or Phenylephrine in APPAMIDE PLUS® Eye Drops (Tropicamide & Phenylephrine Hydrochloride Ophthalmic Solution) is not known whether can cause fetal harm when administered to a pregnant woman or can affect reproduction capacity.

NURSING MOTHERS
It is not known whether these drugs are excreted in human milk. Since most drugs pass into breast milk in small amounts, caution should be exercised when APPAMIDE PLUS® Eye Drops (Tropicamide & Phenylephrine Hydrochloride Ophthalmic Solution) is administered to a nursing woman.

ADVERSE REACTIONS
The following have been reported with ophthalmic use of beta blockers. 
Ocular: Transiently increases IOP in eyes with open-angle glaucoma and may precipitate angle-closure glaucoma in patients with narrow angles, severe contact dermatitis, conjunctival hypoxia.
Common: Allergic reactions.
Cardio Vascular: Hypertension, arrhythmia, syncope, myocardial infraction, tachycardia and subarachnoid hemorrhage. 
Central nervous system : Psychotic reactions, behavior disturbances, sedation, inability to concentrate fatigue in children and some adults.
Respiratory: Cardiorespiratory collapse in children and some adults.

PHARMACOLOGY :

This anticholinergic preparation blocks the responses of the sphincter muscle of the iris and the accommodative muscle of the ciliary body to cholinergic stimulation, producing pupillary dilation (mydriasis) and paralysis of accommodation (cycloplegia). It acts rapidly, but has a shorter duration than atropine.

INDICATIONS AND USAGE:

C-PENT Eye Drops is used to produce mydrisis and cycloplegia. It may be used in the treatment of ititis. iridocycloclitis to prevent the formation of posterior synechiae and rest the painful cilliary and sphincter papillae muscles. It is also useful for refraction studies in children.

C-PENT Eye Drops is used before eye examinations (e.g., refraction exams). It belongs to a class of drugs known as anticholinergics. Cyclopentolate works by temporarily widening (dilating) the pupil of the eye and relaxing the muscles of the eye.

INDICATIONS AND USAGE:

C-ART -Eye Drops (Sodium Carboxy Methyl Cellulose & Glycerin Sterile Ophthalmic Solution), for temporary relief from burning irritation and discomfort due to dryness of the eye or due to exposure to wind or sun. May also be used as a protectant against further irritation.

CONTRAINDICATIONS:

It should not be used when marrow angle glaucoma or anatomical narrow angles are present or where there is hypersensitivity to any component of this preparation.

WARNINGS AND PRECAUTIONS:

To avoid contamination do not touch tip of container to any surface. Replace cap after using. If you experience eyes pain, changes in vision, continue redness or irritation of the eye, or if the conditions worsens or persists for more than 72 hours, discontinue use and consult your doctor.Keep this and all drugs out of reach of children.

Pregnancy:

There are no adequate and well-controlled studies in pregnant women. C-PENT Eye Drops should be used in pregnant women only if the potential benefit to the mother justifies the potential risk to the embryo or fetus.

DOSAGE AND ADMINISTRATION:

Adults

One drop, followed by a second drop in 5 minutes.

Complete recovery usually occurs in 24 hours.

Children

One drop is instilled in each eye, followed 5 minutes later by a second application if necessary.Pretreatment with C-PENT Eye Drops on the day prior to examination usually is not necessary.In order to minimize systemic absorption, apply pressure to the tear duct for two minutes immediately after administration.

INDICATIONS AND USAGE

A-Tropin Eye Drops(Atropine Sulphate Ophthalmic Solution 1% w/v) is a cycloplegic and mydriatic agent indicated for uveitis and refraction.
Geriatrics (>65 years of age):
Elderly patients may be at a higher risk for undiagnosed glaucoma as well as atropine-induced psychotic reactions and behavioural disturbances
Pediatrics (<12 years of age):
A-Tropin Eye Drops(Atropine Sulphate Ophthalmic Solution 1% w/v) is contraindicated in pediatric patients<12 years of age because of the risk of serious systemic side effects.

CONTRAINDICATIONS

A-Tropin Eye Drops(Atropine Sulphate Ophthalmic Solution 1%w/v) is contraindicated in:
-  Patients who are hypersensitive to this drug or to any ingredient in the formulation or component of the container. For a complete listing, see the Dosage Forms, Composition and Packaging sectionof the Prescribing Information.
-  Patients with known or suspected angle-closure glaucoma.
-  Children less than 12 years of age. 
-Children with Down’s syndrome, spastic paralysis or brain damage

WARNINGS AND PRECAUTIONS

General
FOR TOPICAL OPHTHALMIC USE ONLY.
A-Tropin Eye Drops (Atropine Sulphate Ophthalmic Solution 1% w/v) may provoke hyperthermia and should be used with caution in patients, especially children, who may be exposed to elevated environmental temperatures or who are febrile. Atropine may cause drowsiness, blurred vision and sensitivity to light. Patients receiving A-Tropin Eye Drops (Atropine Sulphate Ophthalmic Solution1% w/v) should be advised not to drive or use machinery until their vision clears. Nasolacrimal occlusion or gently closing the eyelid after administration is recommended. This may reduce the systemic absorption of medicinal ingredients administered via the ocular route and result in a decrease in systemic adverse reactions.

Ophthalmologic
A-Tropin Eye Drops (Atropine Sulphate Ophthalmic Solution 1% w/v) may cause increased intraocular pressure (IOP) and provoke glaucoma attacks inpatients predisposed to acute angle closure. The possibility of undiagnosed glaucoma should be considered in some patients, in particular geriatric patients. To avoid glaucoma attacks, IOP and an estimation of the depth of the angle of the anterior chamber should be determined prior to initiation of therapy.

Special Populations
Pregnant Women: There are no or limited amount of data from the use of A-Tropin Eye Drops (Atropine Sulphate Ophthalmic Solution 1%w/v) in pregnant women. There are documented systemic effects stemming from ophthalmic atropine use. A-Tropin Eye Drops (Atropine Sulphate Ophthalmic Solution1% w/v) should only be used to treat pregnant women when the benefit to the mother outweighs the potential risk to the fetus.
Pediatrics (<12 years of age): A-Tropin Eye Drops (Atropine Sulphate Ophthalmic Solution1% w/v) is contraindicated in pediatric patients< 12 years of age.
Pediatrics (≥ 12 years of age): Children, especially premature and low birth weight, or patients with Down syndrome, spastic paralysis or brain damage are particularly susceptible to central nervous system disturbances, cardiopulmonary and gastrointestinal toxicity from systemic absorption of atropine. A-Tropin Eye Drops (Atropine Sulphate Ophthalmic Solution1% w/v) should be used with extreme caution, if at all, in children ≥ 12 years of age with Down syndrome, spastic paralysis or brain damage.
Geriatrics (> 65 years of age): Elderly patients may be at a higher risk of undiagnosed glaucoma as well as atropine-induced psychotic reactions and behavioural disturbances.

ADVERSE REACTIONS

Adverse Drug Reaction Overview
A-Tropin Eye Drops (Atropine Sulphate Ophthalmic Solution1% w/v) produces reactions similarto those of other anticholinergic drugs. Central nervous system manifestations, such as ataxia, incoherent speech, restlessness, hallucinations, hyperactivity, seizures, disorientation as to time and place, and failure to recognize people are possible. Other toxic manifestations of anticholinergic drugs are skin rash, abdominal distension in infants, unusual drowsiness, tachycardia, hyperpyrexia, vasodilation, urinary retention, diminished gastrointestinal motility, and decreased secretion in salivary and sweat glands, pharynx, bronchi and nasal passages. Severe reactions are manifested by hypotension with rapid progressive respiratory depression. Symptoms of toxicity are usually transient (lasting a few hours), but may last up to 24 hours. Prolonged use of mydriatics may produce local irritation characterized by conjunctivitis (follicular), ocular hyperemia, eye edema, eye discharge and eczema.

DOSAGE AND ADMINISTRATION

Recommended Dose
For uveitis: Apply one drop in the eye(s) three times daily.
For cycloplegic refraction: Apply one drop in the eye(s) three times daily for three days. Repeat 1 hour prior to examination.
Missed Dose
In the case of a missed dose, the patient should be advised to take the dose as soon as they remember. However, if it is close to the next scheduled dose, the missed dose should be skipped. A double dose should not be taken to make up for the missed dose.
